Guidelines
Applicants are responsible for gathering all necessary documents and information for approved postings, submitting all articles to Allstate Memorial for review and publication, and may request a memorial for any human fatality that occurs on the state highway or roadway system.
Rules

Only close family can apply (like parents, spouse, siblings, children, in-laws, stepkids, etc.).

You must have approval from all close family. If anyone objects, the memorial will be removed.

No refunds once the memorial is posted.

If it’s a gift, you still need the family’s permission.

You must send an accident report (and possibly a death certificate) to confirm details.

All documents are reviewed within 24–48 hours.

The memorial will be posted and photographed on the Allstate Memorial website.

You can ask to cancel only before it’s posted.

Only one post per victim, but it can include more than one person.

You’ll get an email if your application is approved.

It costs $100, and the payment is non-refundable.
